New students to the district must bring a certified birth certificate. Physical examinations are required for students entering
kindergarten, first, sixth and ninth grades before they can enter school.
If first grade students had a physical for kindergarten another will not be necessary. Kindergarten students entering school for the first time are required to have an eye exam by Oct. 15, 2013.
Book Rental Fees
Book rental fees are collected on an annual basis. Book rental fees for pupils in grades K through eight will be $35 per year. High School textbook fees are $55 plus $5 locker fee - ($60), Driver's Education fee - $50, Parking Sticker Fee $5, Yearbook $40 w/o name, $43 w/name. Yearbook cost will be $45 if ordered after March 15. Book rental fees, if not previously paid, will be due the beginning of school.
Insurance
School time insurance is free to all students. Your child's student insurance packet will be sent home with the students the first day of school. Your child is covered against medical expenses for accidents that occur while attending academic classes during the regular school session against excess medical expenses for an accidental injury. "Excess" and "excess basis" mean benefits under this coverage are paid only after all of your child's other insurance, coverage or benefits have been exhausted. This coverage protection extends to your child's travel time directly to and from your residence to attend regular academic school sessions, up to one hour before or after regular classes. If your child travels to school on a district bus, the coverage extends to the travel time of the bus.
Students no longer have the option of purchasing 24-hour insurance coverage.
Grades 9-12 Tackle Football Coverage may be purchased for $150. No medical exam is required. ISDA specifically designed this optional coverage to provide your child with coverage up to $25,000 in excess benefits resulting from an accidental injury which your 9-12 grade student may suffer while playing interscholastic tackle football sanctioned by IHSA. This coverage also applies to travel directly and uninterruptedly to and from such IHSA sanctioned practices and games, provided the travel is solely organized and scheduled by the school, and supervised by authorized school employees of the school while on or off school premises. This valuable protection begins the later of: (1) the day the payment is received by the ISDA, or (2) the first day of IHSA sanctioned grade 9-12 interscholastic tackle football practice or game. This optional coverage terminates no later than completion of travel from the season's last IHSA sanctioned grade 9-12 interscholastic tackle football game sponsored by your school.
Payment is to be mailed by the parent to Illinois School District Agency (ISDA) - it may be paid with a credit card (may pay online), Check or Money Order. All information for payment is in the insurance pamphlet. As soon as an accident occurs please contact the school office for a claim kit. Parents are responsible when an accident occurs to pick up a claim form from your child's school office immediately - to be sent in by the parent. All instructions are included with the claim form. Please retain your packet of information regarding coverage. There is a Student Accident Excess Coverage Insurance Card in your packet. Cut out the card and keep in a safe place. A certificate of insurance will be sent to you to confirm that the coverage has taken effect and its effective date.
Pay Fees at Time of Registration
Parents are asked to pay all fees as they register, if possible, unless paid at pre-registration. High School students will need (3) separate checks: 1) Parking Sticker Fee - $5 2) Yearbook Fee - $40 w/o name / $43 w/name - (Must be a separate check to DHS yearbook. 3) Book Rental $55 / locker fee $5 - $60 / Drivers Ed - $50.
First Week of School
First day of pupil attendance is Thursday, Aug. 15, 2013.
All schools will dismiss (1) hour early Thursday, Aug. 15, 2013 through Friday, Aug. 23, 2013.
Registration Schedules
Elementary: Kindergarten students who need to turn in physicals, students who did not complete pre-registration, new students to the district, and students who need to pay fees should register on Monday, Aug. 5 from 8:00 a.m. - 3:00 p.m. in the Elementary Media Center. Students who completed pre-registration do not need to attend. Class lists will be posted on the entrance doors on Monday, Aug. 12.
*Back-to-School Night for Du Quoin Elementary School will be Wednesday, August 14, 2013 from 5:00 p.m. to 6:30 p.m. Please take this opportunity to meet your child's teacher, see the location of their room, check out the school, and bring in your child's supplies.
Middle School: Students attending the Du Quoin Middle School who pre-registered (grades 5-8) do not need to register. Students who did not pre-register and also new students should report to school on Monday, Aug. 5 from 8:00 a.m. to 3:00 p.m. Class lists will be posted the week of Aug. 12. The school nurse will be available to review immunization records and physical information. Sixth grade students must present a physical (a sports physical cannot be accepted).
*Students who are new to the district from another school in Illinois must present a Student in Good Standing form from their previous school, an official copy of their birth certificate, and proof of residency. New students from out-of-state must present a physical and immunization records, verification of grade placement, an official copy of their birth certificate, and proof of residency. Students can not complete the registration process until these requirements are met.
*Back-to-School Night for Du Quoin Middle School will be held Monday, Aug. 19, 2013 from 6:00 p.m. - 7:30 p.m. Please take this opportunity to meet your child's teacher, see the location of their room and check out the school.

For students who have not yet been vaccinated and who meet eligibility requirements parents may contact Ellice Wickwire in the immunization department at the Perry County Health Department at 357-5371.
Du Quoin Elementary and Middle School
K through 4th grade will attend Du Quoin Elementary School and 5th through 8th grade will attend Du Quoin Middle School.
Car Riders
In order to assist the flow of traffic in and around the Elementary/Middle School, we are asking those who drive children to school to follow a few simple steps.
Please be aware that Jackson and Line streets are designated as school zones for traffic purposes. Speed limits are posted. The lane directly in front of the school is one way (west to east) and is for bus and staff use only until 7:40 a.m. The west entrance into the school is one way into the lane in front of the school. The middle and east entrances are two way (in and out of school area). Buses will be entering through the west entrance and leaving through the east entrance each morning and afternoon. Those who drop students off in the morning please do not use the front entrance until after 7:40 a.m. each morning. This will give all buses an opportunity to drop off students and leave prior to this time - 7:40 a.m. If you need to drop off students by car before 7:40 a.m. please use the far east entrance, which follows to the rear of the middle school. Pre-K - 8 bus students will be picked up at the end of the day in front of the Elementary/ Middle School and all car riders in grades K - 8 may be picked up in the rear area of the Elementary/Middle School.
Dismissal After School
Kindergarten students who are car riders will be dismissed at 2:55 p.m.
Students in grades 1-2 who are car riders will be dismissed at 3:04 p.m.
Students in grades 3-4 who are car riders will be dismissed at 3:02 p.m.
Middle School students (5-8) who are car riders will be dismissed at 3:05 p.m.
High School students (9-12) will be dismissed at 3:11 p.m.
Walkers
For those students who are walking to school we ask that you use the sidewalks along Jackson and Line Streets. At the corner of Line and Jackson there is a crossing guard to assist students as they cross the intersection from the north. As well, Jackson and Line Streets near the school will be designated as school zones for traffic and pedestrian purposes.
Bus Riders
For those students who ride busses, prior routes that were established will continue to operate with possible modifications. Pick up and drop off points throughout the community have been designated. If a student is bus eligible, school personnel and/or the transportation supervisor will contact parents/guardians pertaining to this issue.

Lunch and breakfast will be served the first day of attendance - Thursday, Aug. 15, 2013. If you need to contact the school regarding the free/reduced lunch program you may call 542-3856 or if long distance you may call collect.
Parents may pay student lunches in advance (weekly, bi-weekly, monthly). Charges are not to exceed $5. Please refer to the handbook.
If you pay lunch money on registration day please send in an envelope with the child's name, grade, and amount of money inside envelope.
**If you received a letter stating that your child(children) are "direct certified" you will not need to fill out a lunch application. Parents will be responsible for lunches purchased up until the date you complete and return your application for the free/reduced lunch program. Please return your application as soon as possible to your child's office.
Food allergies can be life threatening. The risk of accidental exposure to foods can be reduced in the school system if schools work with students, parents and physicians to minimize risks and provide a safe educational environment for food-allergic students. District #300 has worked to make each school building a "peanut product free" environment. We ask that each home respect this effort. If your child has food allergies or other health concerns please fill out the health concerns form in the district handbook you should have already received and return to the school nursing office.
